Citizen Corps Volunteer Registration Form

Thank you for your willingness to volunteer as a member of the Tulsa Mayor's Citizen Corps. Please complete the following information completely, as incomplete information may delay your approval as a volunteer with Citizen Corps or other organizations for which you may be interested in volunteering.

You will need to select a username and password.

You may use any combination of letters and numbers for either of these fields. However, may we suggest that you use at least five letters and two numbers in your password. It is important to select something that you will easily remember as you will need to enter this information each time you access the "For Our Volunteers" area of the site. Do not share your username and password as these will give you access to areas of the site which contain personal information about you, including your social security number.
